Job summary

Low T Center, a leading provider of hormone replacement and targeted wellness services, seeks Mid-Level Providers (NP, PA, APRN, FNP) to support expansion and remotely managed care options. You will work with a physician director to coordinate care for hormone replacement and wellness programs.

The ideal candidate holds an active Illinois NP or PA license, a DEA license, and a Controlled Substances license, with the ability to float between Hoffman Estates, Burr Ridge and Westmont clinics, and

Qualifications

• Must have Active Illinois NP or PA license in good standing.
• Current DEA license.
• Current Control Substance License.
• Must be able to float between Hoffman Estates, Burr Ridge and Westmont Clinic.
• Must be able to train Monday-Friday for 10 consecutive days.

Responsibilities

• Coordinate care with physician medical director and Center Director for hormone replacement and wellness programs.
• Deliver care for hormone replacement and targeted wellness (sleep apnea, weight management, hypertension, low libido).
• Manage staff and operations at the location and ensure high-quality patient care.
