True North Consultants, Inc. is seeking a full-time Advanced Practice Clinician (PA/NP) to provide comprehensive primary care in the Greater Sacramento community health setting.

Compensation ranges from $131,000 to $170,000 annually based on experience; schedule includes 8–5 or a 4‑day 10‑hour option, with multiple local locations.

Qualifications

• Active California Physician Assistant license.
• NCCPA board certification.
• Current DEA certification and BLS certification.
• 1–2 years of clinical experience in community health preferred.
• Strong commitment to serving underserved communities.

Responsibilities

• Perform comprehensive patient evaluations including history and physicals.
• Diagnose and manage acute and chronic conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and interventions within scope of practice.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and modify care plans as needed.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care, health education, and counseling.
• Maintain accurate documentation per agency and regulatory standards.
• Collaborate with physicians and care teams to ensure coordinated, high-quality care.

Job Type: Full-time, Mondays to Fridays 8-5 PM or 4 day (10 hour shift available).

Location: Greater Sacramento area, CA (Multiple locations available)

We are seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care services within a community health center setting. This role offers the opportunity to serve diverse and underserved populations while working collaboratively with physicians and multidisciplinary care teams.

Key Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations


• Diagnose and manage acute and chronic medical conditions


• Order and interpret laboratory tests, imaging, and other diagnostics


• Prescribe medications and therapeutic interventions within scope of practice


• Develop and implement treatment plans in collaboration with the Medical Director


• Monitor patient progress and modify care plans as appropriate


• Initiate emergency procedures when clinically indicated


• Provide preventive care, health education, and counseling services


• Maintain accurate and timely documentation in accordance with agency and regulatory standards


• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care
