Job summary

Chestnut Health is seeking a Physician Assistant to join our physiatry team in Phoenix, AZ. You will collaborate with therapists, nurses, and other clinicians to develop comprehensive rehabilitation plans and improve patient independence.

You will perform patient assessments, monitor progress, and document care in EMRs, while supporting education for patients and families and participating in multidisciplinary activities. Strong interest in Physiatry/PM&R and applicable licenses are required.

Qualifications

• Certified Physician Assistant credential required
• DEA license required on start date
• Controlled Substance license required on start date
• NCCPA certification
• Desire to work in Physiatry/Physical Medicine and Rehabilitation

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation treatment plans
• Conduct thorough patient assessments including histories and physical exams to determine rehab goals
• Monitor progress and adjust treatment plans to optimize functional outcomes
• Participate in multidisciplinary team meetings and educational activities to enhance clinical skills
• Maintain accurate documentation in electronic medical records (initial evaluations, treatment plans, progress notes, discharge summaries)
• Educate patients and families about rehabilitation process, outcomes, and post-discharge care

Essential Duties & Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care

Qualifications

• Actively Licensed to practice in the state of which employed
• Current Controlled Substance License (required upon start date)
• Current DEA License (required upon start date)
• NCCPA Certificate
• Desire to work in Physiatry/Physical Medicine and Rehabilitation
• Orthopedic Experience (nice to have)
• Physiatry Experience (nice to have)
• Skilled Nursing Experience (nice to have)

License/Certification
