Qualifications

• SNF rehab experience preferred.

Responsibilities

• Collaborate with therapists, nurses and other professionals to develop rehab plans.
• Conduct patient assessments and determine rehabilitation goals.
• Monitor progress and adjust plans to promote independence.
• Participate in multidisciplinary meetings and education.
• Document evaluations, plans, progress and discharge summaries in EMR.
• Educate patients and families about rehab process and post‑discharge care.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Qualifications:

• Actively licensed as a Physician Assistant in the state of employment
• Eligible to work in the United States
• Current Controlled Substance License is a plus
• Current DEA license is a plus
• Orthopedic clinical experience is nice to have
• Physiatry clinical experience is nice to have
• Skilled nursing clinical experience is nice to have

License/Certification:

• Active NCCPA certification as a Certified Physician Assistant is a plus
