• NCCPA certification required
• DEA license required upon start date
• Current Controlled Substance license required on start date
• Desire to work in Physiatry/PM&R
• Orthopedic experience is a plus
• Physiatry experience is a plus
• Skilled Nursing experience is a plus

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans
• Conduct thorough patient assessments to determine goals and interventions
• Monitor patient progress and adjust treatment plans to optimize outcomes
• Participate in multidisciplinary meetings and educational activities
• Maintain accurate documentation in EMR systems
• Provide education and counseling to patients and families about rehabilitation and post-discharge care

Essential Duties & Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
