Chestnut Health in Wisconsin is seeking a Physician Assistant to join our Physiatry team. You will collaborate with therapists and nurses to design and implement comprehensive rehabilitation plans aimed at restoring function and improving independence.

Qualifications

• Actively licensed to practice in the state of employment.
• Current Controlled Substance License required on start date.
• Current DEA License required on start date.
• NCCPA certification.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation treatment plans.
• Conduct patient assessments including histories, exams, and diagnostic evaluations to determine rehab goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical skills.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Educate patients and families about rehabilitation processes, expected outcomes, and post-discharge care.

• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
