Job summary

Chestnut Health in Payson, AZ seeks a Physician Assistant to join our physiatry team. You will collaborate with therapists and nurses to plan comprehensive rehabilitation care and conduct thorough patient evaluations to guide interventions aimed at improving function.

You will monitor progress, adjust plans as needed, and participate in team meetings while documenting patient care in EMRs. New grads are encouraged to apply, with a sign-on bonus and strong emphasis on work-life balance.

Qualifications

• Active license to practice in the state of employment.
• Current Controlled Substance License (required on start).
• Current DEA License (required on start).
• NCCPA certification is required.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and healthcare professionals to develop treatment plans.
• Conduct patient assessments including histories, examinations, and evaluations.
• Monitor progress and adjust treatment plans to optimize outcomes.
• Participate in multidisciplinary meetings and educational activities.
• Maintain accurate documentation in electronic medical records.
• Educate patients and families about rehabilitation processes and post-discharge care.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
