Job summary

Chestnut Health in Appleton, WI seeks a Physician Assistant to join the Physiatry team. The role focuses on rehabilitation medicine, improving function and independence through collaborative care with therapists and nurses.

The PA will conduct patient evaluations, develop plans, monitor progress, and educate patients and families about recovery and follow-up care. This position emphasizes work-life balance and a patient-centered approach in a supportive setting.

Qualifications

  • NCCPA Certification

Responsibilities

  • Collaborate with therapists and nurses to create comprehensive rehab plans
  • Perform patient assessments, histories, exams, and determine rehab goals
  • Monitor progress and adjust treatment plans for optimal outcomes
  • Participate in multidisciplinary meetings to enhance clinical skills
  • Document evaluations, plans, progress, and discharge summaries in EMR
  • Educate patients and families about rehab process and post-discharge care

Essential Duties & Responsibilities
  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
Requirements
Qualifications
  • Actively Licensed to practice in the state of which employed
  • Current Controlled Substance License (required upon start date)
  • Current DEA License (required upon start date)
  • NCCPA Certificate
  • Desire to work in Physiatry/Physical Medicine and Rehabilitation
  • Orthopedic Experience (nice to have)
  • Physiatry Experience (nice to have)
  • Skilled Nursing Experience (nice to have)
License/Certification
  • Certified Physician Assistant (Required)
