Job summary

Chestnut Health Company in Kentucky seeks a Physician Assistant to join our Physiatry/Physical Medicine and Rehabilitation team. You will collaborate with therapists, nurses, and physicians to develop comprehensive rehab plans and deliver patient-centered care.

The role emphasizes evaluating patients, documenting findings, and supporting functional outcomes while promoting independence. Active PA licensure in the state and NCCPA certification are required.

Qualifications

• Active license to practice in the state of employment
• Current Controlled Substance license required on start date
• Current DEA license required on start date
• NCCPA certification

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for rehabilitation needs
• Conduct patient assessments, including histories and physical exams, to determine rehabilitation goals and interventions
• Monitor progress and adjust treatment plans to optimize functional outcomes and independence
• Participate in multidisciplinary meetings, case conferences and educational activities
• Maintain accurate documentation in electronic medical records (initial evaluations, treatment plans, progress notes, discharge summaries)
• Educate patients and families about the rehabilitation process and post-discharge care

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care

Requirements
