What You’ll Do:

As an experienced Physician Assistant with Thome PACE, you’ll be part of an interdisciplinary team caring for medically complex older adults. You’ll collaborate closely with nursing, social work, therapy, pharmacy, dietetics, home care, transportation, and scheduling to design the right care plan for each participant. The Physician Assistant supports clinically appropriate, participant centered, and resource responsible care.
