Job summary

Thome PACE in Jackson, MI is seeking an experienced Nurse Practitioner to join an interdisciplinary team caring for medically complex older adults. You’ll collaborate closely with nursing, social work, therapy, pharmacy, dietetics, home care, transportation and scheduling to design the right care plan for each participant.

The Nurse Practitioner supports clinically appropriate, participant-centered, and resource-conscious care.

Schedule: Exempt (salary), min 40 hours per week | Monday - Friday | 8am - 4:30pm | No holidays! No weekends! Department: Thome PACE | Jackson, MI
