Job summary

The Pines at Placerville is seeking a dedicated Physical Therapist Assistant (PTA) to join our rehabilitation team in a skilled nursing setting. You’ll provide hands-on therapy under a Physical Therapist, document progress, and support discharge planning.

Qualifications

  • Licensed Physical Therapist Assistant (PTA) in California (required).
  • Associate degree from an accredited PTA program.
  • Familiarity with EHR systems (Net Health or PointClickCare) is a plus.

Responsibilities

  • Provide therapy treatments to residents under the direction of a Physical Therapist.
  • Accurately document treatments and resident progress in compliance with regulations.
  • Collaborate with the interdisciplinary team on care plans, progress updates, and discharge planning.
  • Educate residents and families on exercises, safety, and fall prevention.
  • Assist with care conferences and rehabilitation meetings.
  • Support safe treatment environments and proper equipment use.
  • Recommend and assist with obtaining adaptive equipment to improve resident mobility and independence.
  • Participate in quality improvement initiatives and staff education as needed
