Job summary

The Pines at Placerville is seeking a licensed Physical Therapist to join our rehab team in Placerville, CA. This role offers both full-time and per diem opportunities with competitive benefits and a focus on resident-centered care.

You will evaluate residents, develop personalized treatment plans, and collaborate with an interdisciplinary team to improve mobility and independence. New grads are welcome to apply.

Qualifications

  • Valid California PTA or PT license in good standing.
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy.
  • New grads encouraged to apply; CPR may be required by licensure.

Responsibilities

  • Evaluate residents and develop individualized therapy plans.
  • Help residents improve mobility, strength, balance, endurance, independence.
  • Collaborate with physicians, nursing, families, and interdisciplinary teams.
  • Supervise PTAs, aides, and students.
  • Participate in care conferences and quality improvement initiatives.
  • Educate residents and families on exercises and fall prevention.
  • Maintain timely documentation compliant with regulations.
  • Assist with restorative nursing programs and staff training.
  • Recommend adaptive equipment and mobility aids.

Responsibilities

Qualifications
  • Licensed Physical Therapist in the State of California
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy
  • Current license in good standing
  • New Grads encourage to apply!
  • CPR Certification (if required by licensure)
  • Skilled in communication, teamwork, and resident-centered care
  • Experience in skilled nursing or rehabilitation settings preferred
