Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving our newly acquired AL/IL facility in a Home Health/Outpatient capacity.

We offer a comprehensive compensation package, including competitive pay of $34-$39 per hour, plus 401k, PTO, medical, dental, and vision benefits. We provide free CEUs, licensure reimbursement after 1 year, and a mentorship & clinical excellence program to support professional growth.

Qualifications

• Licensed PTA; open to therapists at all experience levels.
• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative and medically necessary treatment per requirements under supervision of a Physical Therapist.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides consultation and counseling to patients and families about physical disorders.
• Contributes to discharge summaries per regulatory and clinical requirements.

Experience/Qualifications:

Licensed Physical Therapy Assistant This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.


• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.


• Provides skilled therapy services to patients who need rehabilitation services.


• Assesses & document patient's progress and communicates with supervising Physical Therapist to adjust treatments accordingly.


• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.


• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
