Continuum Rehab Group in Albuquerque, NM is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

You will provide evidence-based, skilled therapy under supervision, document progress, and collaborate with families and staff to enhance residents' recovery and quality of life. The role offers mentorship, professional growth, and a supportive team culture.

Formación

• Licensed Physical Therapy Assistant required.

Responsabilidades

• Utilize evidence-based practice to support interventions.
• Provide skilled therapy under supervision of a Physical Therapist.
• Deliver rehabilitative services to patients with physical functioning disorders.
• Assess patient progress and communicate with supervising PT to adjust treatments.
• Provide consultation to patients and families and other caregivers.
• Assist in discharge planning and documentation as required.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
