Job summary

Sweetwater Care is seeking a Physical Therapist Assistant to support rehabilitation and help patients regain function after illness or injury. You will work under a licensed Physical Therapist to implement individualized therapy plans and document services for accurate billing.

Responsibilities include assisting in quality patient care, following the Plan of Care and guidelines, and coordinating with other disciplines to address patient needs as directed by the supervising therapist.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care of residents who seek short-term/transitional care with the goal to recover and rehabilitate from serious illness, injury or medical event.
• Assist in providing appropriate aspects of quality patient care under the supervision of licensed therapists.
• Document all services provided to ensure accurate and timely billing of therapy services.
• Assures that patient care follows the established Plan of Care, guidelines, meets professional standards, and complies with Federal and State laws.
• Under the supervision of a Physical Therapist, identify and develop a comprehensive plan of care and implement individualized therapy programs.
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient under supervision of a Physical Therapist.
• Other duties and activities may change or be assigned at any time.
