Job summary

Sweetwater Care is seeking a qualified Physical Therapy Assistant to support licensed therapists in helping residents recover and regain independence after illness or injury. You will contribute to therapy programs, assist with mobility, balance, and gait training, and help patients reach their rehabilitation goals.

Under supervision, you will document services, follow the Plan of Care, and coordinate with other disciplines to address patient needs.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care of residents under the supervision of licensed therapists.
• Document all services provided for accurate and timely billing of therapy services.
• Assure patient care follows the Plan of Care, guidelines, standards and regulatory requirements.
• Under supervision of a Physical Therapist, develop and implement individualized therapy programs to restore performance.
• Identify need for and refer to other disciplines to address patient needs.
• Other duties may be assigned.
