Job summary

Sweetwater Care is seeking a Physical Therapist Assistant to support a Physical Therapist in helping patients recover and rehabilitate after illness or injury, aiming to restore functional ability, well-being and independence.

Under supervision, you will assist with quality patient care, document services for billing, follow the Plan of Care, and develop individualized therapy programs to restore mobility and balance, while coordinating referrals when needed.

Qualifications

• High School Diploma required.
• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

Responsibilities

• Provide support care to residents seeking short-term/transitional care to recover and rehabilitate from serious illness, injury, or medical event, regaining functional ability and independence.
• Assist in delivering quality patient care under the supervision of licensed therapists.
• Document all services provided to ensure accurate and timely billing of therapy services.
• Ensure patient care follows the Plan of Care, organizational guidelines, professional standards, and applicable laws.
• Under supervision of a Physical Therapist, identify and develop a comprehensive plan of care and implement individualized therapy programs to restore and enhance performance.
• Identify need for and refer to other disciplines to address patient needs under supervision of a Physical Therapist.
• Other duties may be assigned as needed.
