Job summary

Qualifications

• Active/Valid Illinois PTA license required.
• SNF/Long term care experience preferred.
• Knowledge of current PT treatment practices and PCC is a plus.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist.
• Respond to service requests by relaying information and referrals to PT.
• Implement and modify patient treatment plans under PT supervision.
• Measure and record patient motor function, strength, and muscle performance.
