Job summary

Brushy Creek Post Acute in Greer, SC is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, you will provide skilled therapy to help residents improve mobility, strength, and independence.

We offer a supportive therapy team, flexible PRN scheduling, and a welcoming campus with 12 resident cottages and outdoor spaces. Prior SNF rehab experience and PCC/Casamba familiarity are valued.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Implement individualized treatment plans based on physician orders and therapy goals.
• Monitor resident progress and communicate updates to the Physical Therapist.
• Document treatment sessions, progress notes, and resident outcomes accurately and timely.
• Assist with discharge planning and caregiver education.
• Collaborate with nursing staff and interdisciplinary team members to support positive resident outcomes.
• Participate in care conferences and team meetings as needed.
• Maintain a safe, clean, and organized therapy environment.
• Ensure compliance with all state, federal, and facility regulations.
• Help residents achieve their highest level of function and independence.

Licenses & Certifications

• Current South Carolina Physical Therapist Assistant (PTA) license in good standing
• Must maintain all required continuing education requirements
• Must remain in good standing with the State Board
