Job summary

Empath Home Health is seeking a compassionate PRN Home Health Physical Therapy Assistant (PTA) to support our home-based therapy team in east Pasco County, including Wesley Chapel, Odessa, Zephyrhills, and surrounding areas.

As a PTA working in patients' homes, you will assist the supervising Physical Therapist, participate in patient and family education, and contribute to a holistic care plan with interdisciplinary collaboration.

Qualifications

• Minimum of 1 year experience as a licensed Physical Therapist (PTA).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Provide physical therapy services to patients in compliance with agency policies and procedures, federal and state regulations, following the plan of care under the supervision of a Physical Therapist.
• Assist the Physical Therapist in evaluating equipment needs of patient to increase functional level.
• Participates in the education of patient and family on methods of self-care and techniques to improve or maintain patient’s level of functioning.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health PTA (Physical Therapy Assistant) to work with our Home Health team to deliver individualized care to patients in their homes in east Pasco county (Wesley Chapel, Odessa, Zephyrhills, Land O' Lakes, and surrounding areas).

Locations: Home health visits in east Pasco county (Wesley Chapel, Odessa, Land O' Lakes, Dade City, Zephyrhills, and surrounding areas)

Schedule: PRN role; 5-10+ home health visits per week. Full-time may be available as well!
