Job summary

Empath Health is seeking a compassionate PRN Home Health Physical Therapist to join our Home Health team serving Hernando County, including Spring Hill, Weeki Wachee, Brooksville, and Ridge Manor. You will perform OASIS assessments, develop and revise plans of care, deliver skilled PT services at patients' homes, and educate patients and families on disease processes and self-care.

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT (Physical Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, Ridge Manor, and surrounding areas).

Locations: Home health visits throughout Hernando County (including Spring Hill, Weeki Wachee, Brooksville, Ridge Manor, and surrounding areas)

Schedule: 5-10+ home health visits per week. Required weekend/holiday on-call rotations. Role may expand to part-time or full-time as we grow!

What You'll Need

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
