Job summary

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT Physical Therapist to provide exceptional care in Manatee County (Bradenton, Lakewood Ranch, and surrounding areas).

The role offers flexible scheduling with 5-10+ home visits per week, weekend/holiday on-call rotations, and strong interdisciplinary support to ensure holistic care in patients’ homes. Active Florida PT license and a minimum of 1 year PTA required.

Qualifications

• Active Florida PT license required.
• Minimum of 1 year experience as a licensed Physical Therapist.
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Locations:

Home health visits in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, Ellenton, Palmetto, and surrounding areas). Visits may also be available in Sarasota and/or Hillsborough counties if desired.

Schedule:

PRN; 5-10+ home health visits per week. Required weekend/holiday on-call rotations.
