Job summary

Empath Health in Florida is seeking a compassionate PRN Home Health Physical Therapy Assistant to deliver skilled PT services to patients in their homes across Port Charlotte and south counties. The role supports the Physical Therapist, assists with equipment needs, educates patients and families, and collaborates with an interdisciplinary team to ensure holistic, patient-centered care.

Florida PTA license required; minimum 1 year home health experience preferred; strong assessment and teaching

• Active Florida PTA license.
• Minimum of 1 year experience as a licensed PTA.
• Home health care experience strongly preferred.

Responsibilities

• Provide physical therapy services to patients in their homes under the supervision of a Physical Therapist.
• Assist the Physical Therapist in evaluating equipment needs of patients to increase functional level.
• Educate patients and families on self-care and techniques to improve or maintain functioning.
• Collaborate with the interdisciplinary team to ensure holistic care.
