Job summary

Empath Home Health, part of Empath Health, is seeking a compassionate PRN Home Health Physical Therapist to deliver tailored care to patients in south Sarasota County, including Venice, Nokomis, Osprey, Siesta Key, Gulf Gate and surrounding areas.

We offer flexible scheduling with 5-10+ home health visits per week. Active Florida PT license and at least 1 year of PT experience (home health preferred) are required, with weekend/holiday on-call rotations available.

Qualifications

• Active Florida PT license.
• Minimum 1 year experience as licensed PT.
• Home health care experience preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled physical therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure holistic care.
