Job summary

AdventHealth in Ormond Beach, FL is seeking a Physical Therapy Assistant to work in home health across Palm Coast and Flagler County. This full-time role supports licensed therapists, delivers patient-centered care, and helps with discharge planning and education under supervision.

Responsibilities include documenting progress, coordinating with the care team, maintaining productivity, and adjusting treatment plans to meet patient needs.

Qualifications

• Entry-level competency in physical therapy practice and theory.
• Basic computer skills and proficiency with Microsoft Office.
• Must read, write and speak conversational English.
• 1+ home health experience preferred.
• PTA license is required, BLS certification required, valid driver’s license, and auto insurance.

• Maintains open communication with the case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and appropriate discharge planning.
• Maintains productivity standards and adjusts work hours to meet patient needs.
• Participates in departmental meetings, education sessions, and performance improvement initiatives to maintain clinical competencies.
• Monitors patient response to treatment and communicates with the Physical Therapist for necessary adjustments.
• Educates patients, family members, and caregivers on identified needs and implements treatment plans.
• Documents interventions and patient progress accurately and timely, using measurable terminology.
• Other duties as assigned.

Job Description

Schedule: Full Time

Shift: Monday – Friday, 8am – 5pm

Location: We have need for a PTA to cover the Palm Coast and Flagler County.

Mileage reimbursement provided: $0.76 per mile

Flexible scheduling with Full Time, Part Time and PRN available

• Maintains open communication with the case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and appropriate discharge planning.
• Maintains productivity standards as defined by the department and adjusts work hours to meet the needs of patients.
• Participates in departmental meetings, education sessions, and performance improvement initiatives to maintain clinical competencies and adhere to professional and evidence-based practices.
• Monitors patient response to treatment and communicates with the Physical Therapist for necessary adjustments.
• Educates patients, family members, and caregivers on identified needs, ensuring understanding. Implements treatment plans established by an Physical Therapist utilizing recommended techniques, adhering to insurance guidelines and patient needs.
• Documents interventions and patient progress accurately, completely, and timely, using measurable and objective terminology in line with standards of practice, insurance guidelines, and department/hospital policies and procedures.
• Other duties as assigned.

Knowledge, Skills, And Abilities
