Job summary

Stellar Health Group's Woburn Rehabilitation and Nursing Center is seeking a dedicated Physical Therapy Assistant to join a family-like therapy team. You will deliver day-to-day rehab services on-site in Woburn, following individualized treatment plans and collaborating with therapists and other disciplines.

Qualifications

• Requirements include a current MA State License as a Physical Therapy Assistant and graduation from an APTA-approved PTA program.
• Candidates must comply with state, federal, and APTA standards of practice when planning and carrying out treatment programs.
• Ability to communicate effectively with multiple disciplines to support patient care.

Responsibilities

• Provide day-to-day treatment, including bed mobility, transfers, gait training, stairs, and ambulation on various surfaces.
• Communicate with other disciplines to enhance interdisciplinary team rapport and improve patient care.
• Participate in home visits, family conferences, and community awareness/marketing activities when appropriate.
• Maintain accurate and timely documentation in resident medical records.
• Supervise rehab and restorative aides regarding patient treatments, ROM, ambulation, and positioning of patients.
• Ensure treatment is provided per the plan of treatment and complete all required documentation on time.
• Meet discharge chart standards, including FIMS, home treatment programs, and equipment recommendations following discharge.
