Job summary

CHP Home Care & Hospice is seeking a licensed Physical Therapist Assistant (PTA) to provide rehabilitative services to patients in their homes under a Physical Therapist. You will help implement individualized treatment plans to improve mobility, relieve pain, and enhance function, working closely with patients, caregivers, and the healthcare team in a home setting.

Qualifications

• Experience with clinical assessment documentation and medical terminology.
• Experience in outpatient clinics, inpatient units, SNFs, or home health.
• Knowledge of orthopedics, geriatrics, pediatrics, and acute care.
• Strong communication with patients, families and care teams.
• Experience with discharge planning is an asset.

Responsibilities

What you’ll do

This Physical Therapist Assistant (PTA) position is for Home Health Services, providing rehabilitative services to patients in their homes under the supervision of a licensed Physical Therapist. This role supports the implementation of individualized treatment plans aimed at improving mobility, relieving pain, and enhancing physical function. The PTA works closely with patients, caregivers, and the healthcare team to ensure effective and compassionate care in a home setting.

Duties

• Assist licensed physical therapists in implementing individualized physical therapy treatment plans for patients of all ages
• Perform manual therapy techniques and other therapeutic modalities
• Operate medical equipment safely and effectively
• Document patient progress accurately following clinical assessment documentation and medical documentation standards
• Optimize patient outcomes through effective communication about treatment strategies and patient responses.
• Educate patients and caregivers on exercises, mobility techniques, and injury prevention strategies to promote recovery outside clinical settings.
• Manage electronic health records (EHR) efficiently to ensure seamless documentation and information sharing across the care team.

Experience

• Prior experience in clinical assessment documentation, medical terminology, and operating medical equipment
