Summary

We are hiring a PRN PTA for Home Visits!

The Home Health Physical Therapist Assistant (PTA) performs treatment, provides exercise instruction, and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.

Responsibilities

1. Follow all state-specific laws governing the provision of physical therapy in home care.
2. Perform only procedures qualified and trained for, as defined by the supervising Physical Therapist.
3. Make recommendations for changes in the plan of care to the supervising PT.
4. Instruct and assist patients in evidence-based treatment within the scope of practice and legal regulations.
5. Observe, record, and report patient responses and condition changes to the supervising PT, nurse, and/or physician.
6. Coordinate care with other healthcare team members as appropriate.
7. Educate patients, families, caregivers, and healthcare team members within the scope of practice.
8. Visit patients across the service area and nearby agencies as needed.
9. Complete and submit progress notes per policy for each visit.
10. Maintain schedule flexibility, participate in weekend rotations, and be available on observed holidays to meet patient needs.
11. Participate in staff meetings and committees as requested.
12. Adhere to LHC Therapy Standards of Practice.
13. Perform all other duties as assigned.

Education and Experience

License Requirements

• Current license, registration, or certification to assist in physical therapy practice in the state.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
