Job summary

LifePoint Rehabilitation is seeking a Physical Therapist Assistant for the Saint Joseph Medical Center Acute Inpatient Rehab Unit in Joliet, IL. This full-time role follows a Monday–Friday schedule, 8:00 a.m. to 4:30 p.m., with an hourly wage of $27–$37.

You will implement treatment plans, monitor progress, document outcomes, educate patients and families, and assist with discharge planning and equipment guidance in a dynamic inpatient rehab setting.

Qualifications

• Current state licensure as a Physical Therapy Assistant.
• Previous experience in patient care preferred.
• Current BLS/CPR certification.

Responsibilities

• Implements treatment plan developed by supervising therapist using appropriate modalities.
• Monitors patient's responses to the treatment plan.
• Regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress; Educates patient and family/caregiver.
• Assists with patient discharge planning and information on adaptive equipment and community resources.

How you'll contribute

• Implements treatment plan developed by supervising therapist using appropriate modalities. Seeks consultation as necessary.
• Monitors patient's responses to the treatment plan.
• Regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress. Educates the patient and family/caregiver about patient deficits.
• Assists with patient discharge planning. Provides information regarding appropriate selection/use of adaptive equipment and community support programs.
