Job summary

Lifepoint Rehabilitation in Joliet, IL is seeking a Physical Therapy Assistant to join a dedicated rehabilitation team. You will implement treatment plans under supervision, monitor patient responses, and communicate progress to the supervising therapist.

The role includes documenting services, educating patients and families, and assisting with discharge planning and adaptive equipment choices in a CARF-accredited rehab unit within Saint Joseph Medical Center.

Qualifications

• Current state licensure as a Physical Therapy Assistant.
• Previous experience in the care of patients preferred.
• Current BLS/CPR

Responsibilities

• Implements treatment plan developed by supervising therapist using appropriate modalities. Seeks consultation as necessary.
• Monitors patient's responses to the treatment plan.
• Regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress. Educates the patient and family/caregiver about patient deficits.
• Assists with patient discharge planning. Provides information regarding appropriate selection/use of adaptive equipment and community support programs.
