Job summary

Innovate Rehab and Wellness in Missouri is seeking a licensed Physical Therapy Assistant (PRN) to join our Skilled Nursing Facility team. You will work under a licensed PT to help patients improve mobility, regain strength, and increase function through individualized treatment plans.

This PRN role emphasizes one-on-one patient care, flexible scheduling, and a supportive, veteran- and therapist-owned culture.

Qualifications

• Associate degree from an accredited PTA program.
• Valid state PTA license.
• Rehab/clinical experience preferred; new grads welcome.
• Strong communication and interpersonal skills.
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate, compliant therapy records and collaborate with the care team.
