Job summary

Innovate Rehab and Wellness in Missouri is seeking a Licensed Physical Therapist for a PRN role in a Skilled Nursing Facility. The position offers flexible, as-needed scheduling to support work-life balance, with a focus on one-on-one patient care and a supportive, therapist- and veteran-owned culture.

Evaluate patients and create individualized treatment plans. As part of an interdisciplinary team, you will perform comprehensive evaluations, implement evidence-based treatments, educate patients

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
