Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant (PRN) to support patients in a Skilled Nursing Facility. You will work under a licensed Physical Therapist to deliver individualized, hands-on therapy aimed at improving mobility, strength, and function in a patient-centered environment.

Qualifications

• Associate degree from an accredited PTA program is required.
• Valid state PTA license is required.
• Rehab/clinical experience is preferred; new grads welcome.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
