Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant (PTA) for full-time work in a skilled nursing facility. You will work under a licensed PT to help patients improve mobility, regain strength, and increase function, delivering individualized, hands-on care in a collaborative team setting.

Responsibilities

• Lead patients through exercises, mobility training, and rehab activities
• Educate patients on safe techniques and assistive devices
• Monitor and document patient progress; report changes to the PT
• Maintain accurate therapy records
• Collaborate with the care team for coordinated treatment
• Uphold ethical, professional standards in all interactions
