Job summary

Holland Hospital seeks a Physical Therapist Assistant to join our outpatient Rehabilitation team in the greater Holland area. Physical Therapist Assistant

Holland Hospital Physical Therapy and Rehab teams value our patients and embrace our organization’s positive behavioral standards in order to provide high‑quality patient care. More than 50% of our rehab team has been with the organization for 14 years or more. We have 5 outpatient clinics located throughout the greater Holland area. Our clinicians typically see 8‑10 patients per day with direct 1:1 care. We provide hourly time slots for evaluations and treatment sessions are generally 45 minutes.

Responsibilities

The physical therapist assistant treats patients under the direction of the licensed therapist and according to the established treatment plan, supervises and delegates tasks to supportive personnel, maintains appropriate records, communicates patient progress to physicians and other health care workers as indicated, meets department productivity expectations, accepts responsibility for personal and professional growth and performs other duties as assigned. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.

Demonstrates essential skill in the use of treatment modalities, equipment and protocols. Performs reassessments as assigned and according to state and facility guidelines, adjusting treatment plan accordingly: selects treatment methods appropriate to the patient’s diagnosis, age group, physical and emotional status as evidenced by observation and documentation. Recognizes the need for alterations in the treatment method and overall plan as the patient advances or regresses, and acts accordingly as evidenced by observation, documentation and team feedback.

Notifies primary PT/ OT of non‑routine changes in the treatment situation, discharge complications or other circumstances that require reassessment or staff guidance as evidenced by team feedback. Instructs ancillary staff in patient positioning, mobility and care giving as evidenced by documentation.

Educates patient and/or family in home exercise programs, transfers, caregiving, home safety and use of adaptive equipment, appropriate to level of understanding.

Adheres to department policy regarding time frame, appropriate form and content of written rehabilitation documentation: completes daily progress notes that reflect patient’s response to treatment, actual performance, and an assessment as to status, all relevant to the treatment plan and eventual discharge needs. Completes treatment summaries when applicable. Documents reasons for canceled treatments. Documents clearly, concisely and legibly within established time frames (e.g. allows other therapists to continue treatment, allows for interpretation by payers, supports reimbursement). Documents evidence of patient and family/care education as evidenced by chart review.

Communicates effectively with patients, family, medical staff, care team members, students, supervisors and outside contacts: contacts families, physicians or other health care team members in anticipation of special needs of the patients or family in a timely manner to facilitate effective treatment plans. Provides educational materials and methods appropriate to the patient’s level of understanding and motivation. Includes family/care giver in education and training as appropriate as evidenced by chart review. Demonstrates collaboration in communication with team members and is open to feedback and receptive to change as evidenced by team feedback. Gives appropriate instruction and supervision to assistive personnel as evidenced by team feedback.

Completes all administrative and other responsibilities in support of the operating unit: maintains productivity standards set by department/clinic. Makes efficient use of non‑billable time and minimizes use of non‑billable time. Completes routine administrative responsibilities within established time frames consistently (e.g. TACS, charge sheets, productivity logs, and other clerical duties as requested). Contributes to educational and training efforts of department/clinic (e.g. in‑services, students, new hire orientation) as evidenced by program‑specific documentation. Demonstrates fiscal responsibility and knowledge about department expenses and revenue (e.g. supplies, equipment, billing, and staff resources) used in the department as evidenced by team feedback and observation.
