Continuum Rehab Group in Kirkwood, MO is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long-Term Care Facilities. You will provide rehabilitative services and collaborate with Physical Therapists to promote patient recovery and quality of life.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients under supervision of a Physical Therapist.
• Provides skilled therapy services to patients in need of rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients and families related to physical disorders.
• Provides input to supervisor for discharge summaries of services provided per regulatory and clinical requirements.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
