Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. We offer mentorship and a robust compensation package with licensure reimbursement and paid CEUs.

This role supports evidence-based rehabilitation under the supervision of a Physical Therapist, delivering skilled services and documenting patient progress to optimize recovery and quality of life for residents.

Qualifications

• Licensed Physical Therapy Assistant credential required.
• Open to therapists at all experience levels.
• Under supervision of a Physical Therapist, provide rehab interventions and assessments.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative and medically necessary treatment under supervision of a Physical Therapist.
• Delivers skilled therapy services to patients needing rehabilitation.
• Assesses patient progress and communicates with supervising PT to adjust treatments.
• Provides counseling to patients and families regarding physical disorders.
• Contributes discharge summaries per regulatory and clinical requirements.

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.

• Provides skilled therapy services to patients who need rehabilitation services.

• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
