Job summary

Continuum Rehab Group in Independence, MO seeks a licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities. We offer a comprehensive compensation package and benefits, mentorship, and opportunities for specialty certification paid by CRG.

Licensed PTA will provide evidence-based, skilled therapy under the supervision of a Physical Therapist and contribute to improving residents' recovery and quality of life.

Qualifications

• Licensed Physical Therapy Assistant certification required.
• Experience providing rehabilitative therapy in SNF/long-term care preferred.
• Under supervision of Physical Therapist, deliver skilled therapy services.

Responsibilities

• Utilizes evidence-based practices to guide interventions.
• Assess and document patient progress; communicate with supervising PT.
• Provide skilled therapy to patients needing rehabilitation services.
• Counsel patients, families, caregivers, and other service providers.
• Assist in discharge planning and summaries as required.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.


• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.


• Provides skilled therapy services to patients who need rehabilitation services.


• Assesses and document patient's progress and communicates with supervising Physical Therapist to adjust treatments accordingly.


• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.


• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
