Continuum Rehab Group seeks a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Herculaneum, MO.

This role supports compassionate patient care, supervision under a Physical Therapist, and promotes residents’ mobility and quality of life within a supportive, clinical environment.

Qualifications

• Licensed Physical Therapy Assistant credential required.
• 1+ years of relevant clinical experience preferred.
• Support clinicians with evidence-based interventions.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising PT to adjust treatments.
• Provides consultation and counseling to patients and families related to physical disorders.
• Contributes to discharge summaries per regulatory requirements.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.

• Provides skilled therapy services to patients who need rehabilitation services.

• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
