Job summary

Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing and Long Term Care facilities. You will work closely with licensed Physical Therapists to develop and implement rehab plans that improve residents’ mobility and quality of life in a compassionate, team-oriented setting.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Under supervision of the Physical Therapist, provides rehabilitative, skilled and medically necessary treatment.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising PT to adjust treatments.
• Provides counseling to patients and families, caregivers and other providers about physical disorders.
• Contributes discharge summary input per regulatory and clinical requirements.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
