Job summary

HealthPro Heritage is seeking a compassionate Physical Therapist Assistant for a full-time role at Masonicare in Wallingford, CT. You will participate in implementing treatment plans, assist with exercises, and support patients across a CCRC setting.

Qualifications

• Associate’s degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and teamwork.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists and provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
