Connect Pediatrics is seeking a Pediatric Physical Therapist Assistant to join our in-home care team serving Little Elm, Aubrey, and Frisco, TX. You will implement pediatric treatment plans in patients’ homes, collaborate with physicians and nurses, and communicate progress with families and the wider care team.

The ideal candidate holds a Texas PTA license, is CPR/BLS certified, and has reliable transportation to travel between patient homes.

Qualifications

• Active Physical Therapist Assistant license in Texas.
• Current BLS/CPR certification.
• Reliable transportation and valid driver’s license.

Responsibilities

• Implement in-home pediatric treatment plans per physician orders.
• Provide in-home therapy for a range of pediatric diagnoses.
• Engage with families, updating goals and progress.
• Collaborate with a multidisciplinary team to coordinate care.
• Maintain thorough documentation and communicate changes in treatment plans.

Responsibilities

• Implement relevant treatment plans aligned with physician's orders.
• Provide in-home therapy services for pediatric patients with a wide range of diagnoses.
• Engage with patients' families, providing them with regular communication and updates on goals for their child.
• Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient
• Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.
• Actively participate in one-on-one meetings with PT Supervisor/team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.
