Job summary

Lincoln Post Acute is seeking a Physical Therapy Assistant (PTA) to join our rehabilitation department, helping residents regain independence and mobility. We offer Full-Time, Part-Time, and Per-Diem opportunities with competitive pay and a supportive team focused on exceptional patient care.

You'll work with an interdisciplinary team to develop and implement individualized treatment plans, monitor progress, and educate residents and families on exercises and safety techniques.

Qualifications

  • Active California Physical Therapist Assistant License or eligible
  • Graduate of an accredited PTA program
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team-oriented environment
  • Familiarity with EMR systems (PointClickCare/Net Health) preferred

Responsibilities

  • Provide physical therapy treatments under supervision of a licensed PT
  • Assist residents in achieving functional goals and improved quality of life
  • Implement individualized treatment plans
  • Monitor resident progress and communicate updates to the therapy team
  • Educate residents and families on exercises and mobility techniques
  • Maintain accurate documentation per facility policies and regulatory requirements
  • Collaborate with nursing and therapy teams to deliver exceptional resident care
  • Bring compassion, energy, and professionalism to every patient interaction

What You'll Be Doing

As a Physical Therapist Assistant, you'll play a key role in helping residents improve mobility, regain strength, and maximize independence.

Responsibilities Include
  • Providing physical therapy treatments under the supervision of a licensed Physical Therapist
  • Assisting residents in achieving functional goals and improved quality of life
  • Implementing individualized treatment plans
  • Monitoring resident progress and communicating updates to the therapy team
  • Educating residents and families on exercises, safety techniques, and functional mobility
  • Maintaining accurate and timely documentation in accordance with facility policies and regulatory requirements
  • Collaborating with nursing, therapy, and interdisciplinary team members to provide exceptional resident care
  • Bringing positive energy, compassion, and professionalism to every patient interaction
What We're Looking For

Qualifications:

  • Active California Physical Therapist Assistant License (or eligible to obtain)
  • Graduate of an accredited Physical Therapist Assistant program
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team-oriented environment
  • Passion for helping others achieve their rehabilitation goals
  • Experience in skilled nursing or post-acute care is a plus, but not required
  • Familiarity with PointClickCare, Net Health, or similar EMR systems is preferred
