Midtown Oaks Post Acute is seeking a motivated and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a part-time basis in Sacramento, CA.

In this role you will provide PTA-supervised treatments, help patients regain mobility, balance and independence, and participate in discharge planning within a supportive, multidisciplinary environment. Qualifications

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
• Assist patients in improving mobility, balance, strength, endurance, and functional independence
• Implement individualized treatment plans and therapeutic exercise programs
• Accurately document treatments, patient responses, and progress toward goals
• Collaborate with therapists, nursing staff, physicians, residents, and families to support optimal outcomes
• Participate in discharge planning and continuity of care initiatives
• Maintain compliance with all facility, state, and federal regulations

Qualifications

• Current California Physical Therapist Assistant (PTA) license
• Graduate of an accredited Physical Therapist Assistant program
• Skilled nursing, post-acute, rehabilitation, or long-term care experience preferred
• Strong clinical, communication, and documentation skills
• Compassionate, patient-centered approach to care
• Ability to work effectively in a multidisciplinary team environment
