Job summary

Transitions At Home for Western Wisconsin is seeking a full-time Home Health Physical Therapist to provide skilled therapy in patients' homes, developing individualized care plans with physician orders and collaborating with the interdisciplinary team.

You will deliver treatments, educate patients and families, and support mobility and independence in a home-care setting, with opportunities for professional growth and on‑the‑job training.

Qualifications

• Active Wisconsin Physical Therapist license is required.
• CPR certification is required.
• Valid Wisconsin driver's license and car insurance are required.
• Comfortable working independently in a home-care environment and collaborating with care teams.

Responsibilities

• Develop and revise care plans.
• Perform skilled physical therapy procedures.
• Educate patients and families on home programs and daily activities.
• Collaborate with physicians and the care team.
• Participate in IDT meetings, in-service training, and QA initiatives.

Your Role:
As a Home Health Physical Therapist, you’ll deliver skilled therapy services based on individualized care plans and physician orders. You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.
