Millennium Physician Group in Fort Myers, FL, seeks a licensed Physical Therapist Assistant (PTA) to join our Home Health Team serving Lee County. You will evaluate patients, develop individualized plans, and provide interventions to improve mobility, strength, balance, and independence at patients’ homes.

Collaborating with an interdisciplinary team, you’ll educate families, maintain documentation, and ensure regulatory compliance to deliver high-quality care.

Qualifications

• Current Florida PTA license required.
• Minimum 6 months of hospital, skilled nursing facility, or rehab experience.
• Excellent communication, interpersonal, and critical thinking skills required.
• Strong work ethic, reliability, and commitment to quality care.
• Reliable transportation to visit patients in their homes.

Responsibilities

• Provide skilled therapy services under the supervision of a Physical Therapist.
• Implement treatment plans including therapeutic exercises, gait training, and assistive device training.
• Educate patients and families on techniques to improve functional skills and independence.
• Collaborate with Physical Therapists, occupational therapists, and other healthcare professionals to ensure optimal outcomes.
• Maintain accurate and timely documentation of patient progress, plans, and interventions.
• Adhere to infection control guidelines to protect patients and healthcare workers.

What You’ll Do

Compensation Range:

$19.00

to

$28.50

The anticipated base salary range represents the Company’s good‑faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
