Job summary

Millennium Physician Group is seeking a Physical Therapy Assistant to join our Home Health Team serving Lee County, FL. You will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.

You will collaborate with an interdisciplinary team, educate patients and families on rehabilitation strategies, and maintain compliant clinical documentation while upholding infection control standards.

Qualifications

• Current and valid Florida PTA license.
• Minimum of 6 months experience in hospital, skilled nursing facility, or rehab.
• Excellent communication, interpersonal, and critical thinking skills.
• Reliable transportation to visit patients at home.

Responsibilities

• Provide skilled therapy services under the supervision of a Physical Therapist.
• Implement treatment plans, including therapeutic exercises, gait training, and assistive device training.
• Educate patients and families on techniques to improve functional skills and independence.
• Collaborate with Physical Therapists, occupational therapists, and other healthcare professionals to ensure optimal patient outcomes.
• Maintain accurate and timely documentation of patient progress, treatment plans, and interventions.
• Adhere to infection control guidelines to protect patients and staff.
