Job summary

Millennium Physician Group is seeking a Physical Therapist (PT) to join our Home Health Team in Sarasota County on a PRN basis. You will evaluate patients in their homes, develop individualized treatment plans, and provide interventions to improve mobility, strength, balance, and independence.

Work closely with an interdisciplinary team, educate patients and families, and ensure accurate documentation and regulatory compliance.

Qualifications

• Current and valid Physical Therapist license in Florida.
• Minimum 1 year of clinical PT experience, preferably home health.
• Strong clinical, communication, and patient-centered care skills.
• Professionalism, reliability, and transportation to visit patients at home.

Responsibilities

• Perform comprehensive patient assessments and develop individualized treatment plans.
• Apply therapeutic interventions including manual therapy and therapeutic exercises; gait training.
• Educate patients and families on body mechanics, home exercise programs, and prevention.
• Collaborate with physicians, nurses, and other therapists to ensure integrated care.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharges.
• Adhere to HIPAA and Medicare regulations and other regulatory standards.

How will you make an impact & Requirements

What You’ll Do

• Patient Assessment: Conduct comprehensive evaluations of patients’ physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients’ mobility, strength, balance, and pain management.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long‑term health and well‑being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We’re Looking For

• Licensure: Current and valid Physical Therapist license in the state of Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
• Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
• Transportation: Reliable transportation to visit patients in their homes.
