Job summary

Millennium Physician Group in Florida is seeking a Licensed Physical Therapist to join our Home Health Team serving Collier County. You will evaluate patients in their homes, develop individualized treatment plans, and provide interventions to improve mobility, strength, balance, and independence, while educating families and maintaining compliant clinical documentation.

The role collaborates with an interdisciplinary team to ensure quality care; requires licensure in Florida and at least 1 year

Qualifications

• Florida licensed Physical Therapist (PT) in good standing
• Minimum 1 year of clinical PT experience, preferably in home health
• Strong interpersonal and communication skills
• Patient-centered approach to care
• Reliable transportation to visit patients at home

Responsibilities

• Evaluate patients in their homes and perform comprehensive PT assessments
• Develop individualized treatment plans and implement therapeutic interventions to improve mobility, strength, balance and independence
• Educate patients and families on home exercise programs, body mechanics, and injury prevention
• Collaborate with physicians, nurses, occupational therapists and other team members to coordinate care
• Maintain accurate documentation of evaluations, plans, progress notes and discharges
• Adhere to HIPAA and Medicare regulations and ensure regulatory compliance

How Will You Make An Impact & Requirements

What You'll Do

• Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well-being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We're Looking For

• Licensure: Current and valid Physical Therapist license in the state of Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
• Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
• Transportation: Reliable transportation to visit patients in their homes.

Compensation Range

$81,492.00 to $122,238.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
